Expansion - The Future Agape Village

Agape Network is a fully coordinated and integrated community health center that is nationally accredited by CARF. Agape’s mission is to “Restore individuals and families to psychological, social, physical, and spiritual health”. Agape’s patient centered medical health home includes multiple programs to provide a continuum of care with a holistic approach for all clients and their families. Agape recognizes that long-term wellness and recovery from mental illness, substance use disorders, criminality, trafficking, and severe trauma requires alternative programming to traditional models of treatment and rehabilitation. Agape’s evidence based practices incorporate a diverse overlay of programs that include: Vegetable & Wellness Plant Nursery, Culinary Arts, Dance, Music, Arts, Theater, and Equine Therapy. This helps Agape further individualize the client treatment plan for long term recovery that helps them and their children reach their full God given potential.

 

Today over 1,000 mothers, children and families receive weekly services through Agapes Residential Inpatient and Outpatient Departments. Agape is a non-profit and non-denominational community health center that includes residential programs and services for underserved at risk women, including pregnant and postpartum mothers with children, diverting them from the foster care system, criminal justice, and state operated facilities. 

Agape is looking for partners to join in the expansion of the “Agape Village Health Center” expanding capacity from 62 to 141-beds with an Educational & Vocational Center, Clinic & Wellness Medical Center, Transitional Supported Housing Units/Cottages, and a Preschool/Early Education/Day Care Center. The “Agape Village Health Center” expansion has been approved in 2001, by the Commissioners and the Building & Zoning Department under Resolution #CZAB15-16-01. 
 

Agape Network is uniquely positioned and qualified to serve difficult patients with complex behavioral and psychical health care needs. Agape care teams include medical directors, psychiatrists, advanced registered nurse practitioners, nurses, therapists, licensed clinical social workers, care/case managers, care coordinators, chaplains, DCF/AHCA on-site benefits eligibility determination specialist, intake team, registered dietician, and peer specialists. Agape provides various specialized programs including pregnant and post-partum women and their children, outpatient and inpatient residential services, infant/children’s mental health, adult mental health, chemical dependency, substance use disorders, and hospitalist network support.

 

Agape care management teams have an emphasis on being whole person and family centered, engaging and enhancing patient’s natural support system across the spectrum of physical/behavioral health services, social services, housing, prevention, education, and employment. Agape uses mobile and telehealth capabilities for real time in-home continuity of care.

 

In partnership with Health Choice Network (HCN), Agape is part of a national model of successful collaboration among community health partners. By sharing common infrastructure, population health technology, electronic health records, risk management, managed care, value based quality care, research and administrative services, members increase effectiveness, decrease costs, and provide better, more consistent, coordinated and accessible care. Agape provides a comprehensive care management solution with population health technology, helping coordinate care transitions, identify and manage high-risk patients, and close care gaps. Through this population health care management solution our Agape Village Health Center and Community Care Teams will have:

                                     

  • Access to data aggregation throughout the healthcare continuum (EMR, claims, labs, etc.) real time ADT data to drive transition of care.

  • Assessment process driving personalized care plan and compliance.

  • Communication throughout care team members and patients.

  • Workflow development supporting proactive predictive analytics.

  • Accountability by analyzing and reporting the effectiveness of care.

  • Detection of incomplete or missing data in transitions that are relevant to the care continuum.

  • Enhanced focus on prevention, early identification and management of chronic health problems.

 

The “Agape Village Health Center” will improve the health and wellness of our communities with a holistic approach that includes linking clients to a patient centered medical health home for long term wellness and preventive care.
